To support our vision, local health and care services are working together as equal partners to support seamless, person-centred care and tackle health inequalities by improving the lives of the poorest fastest.

All Age Continuing Care (AACC) is delivered across nine localities that include:

  • Sefton
  • Liverpool
  • Knowsley
  • St Helens
  • Warrington
  • Halton
  • Cheshire East
  • Cheshire West
  • Wirral

Advertised roles also include the opportunity for flexible working arrangements.

The team deliver a needs-led process of assessment, review, and provision of appropriate packages of care, either in residential settings or the individual’s home environment. Our place-based delivery teams are responsible for the assessment, commissioning and review of all age continuing care, this includes Continuing Healthcare (known as CHC) but also includes:

  • Funded Nursing Care
  • Mental Health (including Section 117 aftercare)
  • Learning Disability
  • Children and Young People’s Continuing Care
  • Jointly Funded Packages of Care
