Calling all nurses

All Age Continuing Care (AACC) is a collective term for services provided by the NHS. These services assess and provide funding for the care of individuals of all ages to meet their ongoing health and care needs.

Cheshire and Merseyside Integrated Care Board (ICB) serves a diverse population of over 2.5 million people across nine boroughs, working in close partnership with local authorities, 17 NHS provider organisations, 51 Primary Care Networks (PCNs), the North West Ambulance Service, as well as third-sector and voluntary organisations. As the third-largest Integrated Care System (ICS) in the country, Cheshire and Merseyside’s breadth of organisations is double the size of the next largest ICS, making it a uniquely complex and collaborative healthcare system.

A third of our population lives in the most deprived 20% of neighbourhoods in England, and overall, Cheshire and Merseyside experiences some of the poorest health outcomes in the country. Addressing these disparities is at the core of our mission.

Whether through short-term targeted interventions or long-term transformational support, we work with our partners to drive meaningful change.

At the heart of our AACC, which oversees the assessment process for NHS Continuing Healthcare (CHC) and NHS-funded Nursing Care. NHS Continuing Healthcare is available for individuals with a primary health need, ensuring they receive the necessary care and support at no cost.

Registered nurse qualifications (NMC)

Educated to degree level or equivalent qualification/experience

Familiar with the National Framework for NHS Continuing Healthcare and funded nursing care

Great communicator with excellent interpersonal skills
