You will play a key role in supporting the AACC service working within the remit of the National Framework for Continuing Healthcare and funded-Nursing Care, delivering an effective, consistent, competent and person centred care.

The post holder will be responsible for assessing individuals, ensuring the National Framework for NHS Continuing Healthcare and Funded-Nursing Care is implemented and complied with according to the principles and processes. This includes management of packages of care, ensuring these are commissioned to meet identified health outcomes.

You will support personalised care by offering personal health budgets and help individuals and their families to understand these.

Band 5 Associate Commissioning Nurse

As a very brief overview, you will

  • be the first point of contact for referrals into the service and co-ordinating assessments for NHS Funded Nursing Care and NHS Continuing Healthcare as close to the service user as feasible.
  • provide clinical commissioning oversight to applications and assessments concerning joint funded packages of care
  • review service user eligibility for CHC, NHS funded care in accordance with National Framework for NHS Funded Care, and Joint funded care packages
  • undertake urgent reviews of service users, where quality and safety concerns have been identified and report in a timely manner
  • prepare and complete comprehensive documentation/tools to enable the level of NHS funding to be determined within the parameters of the National Framework for Continuing Healthcare and Funded Nursing Care and Joint Funding arrangements locally

Band 6 Commissioning Nurse for

As a very brief overview, your role will be to work closely with acute Trusts to support discharge to assess pathways, completing all reviews and assessments in line with the service’s key performance indicators.

You will:

  • deliver optimum holistic care to service users identified for NHS healthcare funding
  • be actively involved in ensuring that service users meet the eligibility criteria for NHS Continuing Healthcare
  • ensure service users are reviewed in line with national and local policies and procedures, as well as those eligible for Section 117.
  • assess, commission, plan and procure packages of care for service users in line with the AACC Commissioning Policy.
